Background

Phenibut, also known as β-Phenyl-γ-aminobutyric acid or noofen, has been used significantly in Russia since the 1960s as a nootropic supplement for reducing anxiety and stress, for supporting a feeling of well-being and positive, sociable mood, and to promote good restful sleep. GABA is the primary inhibitory neurotransmitter molecule in the brain, and GABA receptors are divided into three groups: GABA-A, -B, and –C.  Each GABA receptor type has different functions. Targeting GABA-A subtypes produces sedative, anxiolytic, muscle-relaxing, and alcohol-potentiating effects, as well as potential nootropic effects (for antagonists or negative allosteric modulators of GABA-A subtype 5). Targeting GABA-B subtypes reduces pain signals, relieves muscle tension and spasticity, and in low or threshold amounts can have positive effects on memory, learning and mood, and some research suggests GABA-B agonists may support sociability and help those with Autism or Asperger’s. Phenibut (or fenibut / phenybut) acts primarily as a GABA-B agonist and in larger amounts may also play a role in activating some GABA-A receptor. Its use as a GABA agonist is important because it is not efficient to supplement with GABA alone due to the chemical structure of the GABA molecule. The phenyl group on Phenibut allows the chemical to cross the blood-brain barrier, while GABA itself cannot do this.  It should be noted that other commonly used substances are also GABA agonists, which include alcohol as well as commonly prescribed anti-anxiety and sleep-inducing drugs.[1]


Effects & benefits

GABA receptors are found throughout the CNS and specifically in regions of the spinal cord that are known to be associated with pain. The GABA receptors are also found outside of the spinal cord and in areas of the CNS that are known to coordinate the response to and perception of pain, as well as muscle tension and spasticity. GABA is an inhibitory neurotransmitter which acts to regulate (‘filter out’) the amount of information that reaches the CNS. In a very simplified explanation, GABA transmitters are able to modulate the perception of pain and since they are inhibitory, a greater concentration of GABA will inhibit the perception of pain and reduce muscle tension and spasticity. Since Phenibut acts as a GABA-B agonist, it is also able to reduce pain and muscle tension, along with other anxiolytic effects that include mood-lift and a calm sense of well-being, sometimes perceived as euphoria.[2]

GABA-B receptors are also being studied in the treatment of Autism and Asperger’s diseases, where the GABA-B receptor agonist arbaclofen showed some promise in enabling greater social function.  However it is important to note that Phenibut is sold as an amino acid supplement and is not intended to not intended to diagnose, treat, cure, or prevent any disease.

One benefit of Phenibut is that the serving size can be altered according to what effect is desired. For simple anxiety relief and mood lift, a lower serving size (closer to 500mg) is used. When the serving size is increased, Phenibut can begin to act more like a sedating or hypnotic (sleep-inducing) supplement.

The spectrum of effects at different serving sizes may emerge from the nature of GABA’s actions at various receptors, and the degree to which different concentrations of Phenibut serve to activate an increasing set of GABA receptors.  Activating different GABA-receptors subtypes have different effects on the human body. While GABA-B agonists have a strong potential to aid pain and anxiety relief, at higher serving sizes they may possibly begin to activate additional GABA-receptor groups, leading to sedating and sleep-promoting effects.

Often when we lie awake at night we are thinking about a lot of things and feeling a lot of things in our bodies and are generally restless. Phenibut is able to block out these perceptions and calm the mind to improve sleep quality.[3]

A lot of research has gone into the use of Phenibut as a neuroprotective agent, and it has been found to be able to protect the brain from stress, especially when the brain is deprived of oxygen, such as can occur during drowning, injury, or during extreme physical exertion which could cause overheating.

Phenibut has also been found to have profound cardioprotective effects, which include protecting the heart from injury under stressful conditions.

Scientists have concluded from these studies that Phenibut results in higher cardiac contraction and relaxation rates, higher left-ventricular pressure during functional tests, and increased indexes of oxidative phosphorylation.”[4]


Phenibut recommended usage

The recommended serving size of Phenibut is 500-2000mg per day, optionally in two to three separate servings, and NEVER more than 4 grams (4000mg) total per week. Phenibut is best used when taken only if needed for a specific purpose, or cycled and taken not more than two days per week.  If taken continuously, Phenibut must be used at a much lower daily serving size to be sustainable.  A lower Phenibut serving size usually encourages mood-lift, anxiety relief, sociability and motivation, whereas in higher doses, Phenibut acts more as a sedating supplement and results in better, deeper sleep. It is strongly recommended NOT to exceed the daily or weekly maximum recommended consumption of Phenibut, as negative effects can occur when too much Phenibut is consumed. 


Phenibut side effects and warnings

Phenibut is for many people an extremely useful supplement with multiple calming and mood-enhancing effects - but, as with all GABA agonists, it can have notable side effects, especially if not used with respect and caution.

Phenibut’s side effects when taken within the recommended serving sizes are generally mild or manageable, but can include gastrointestinal issues, dizziness, tiredness, memory reduction and lethargy, which are common with the use of various GABA-agonists.[5]

Withdrawal effects from Phenibut use have also frequently been reported. For this reason, when discontinuing Phenibut, it is recommended to steadily reduce dosage of Phenibut before stopping entirely.  Stopping Phenibut consumption abruptly, especially if taking Phenibut near or above the maximum recommended amounts, can cause negative withdrawal symptoms.  Phenibut withdrawal effects can include negative thoughts, depression, lethargy, prolonged insomnia, persistent restlessness, anxiety and panic attacks. There is a reported case of a 25-year old man in Russia who became hospitalised for psychosis from withdrawal after long high-dose use of Phenibut.[6] The man was consuming a Phenibut dose of 20 grams of Phenibut per day, which is a huge amount and creates massive risks of overdosing and withdrawal, as well as placing stress on the liver. The symptoms he experienced had some similarities to alcohol-dependant withdrawal as well as withdrawal from Baclofen, GHB, and benzodiazepines which are also GABA agonists.
